We are looking for a skilled individual who is passionate about the work of the not-for-profit and health sector to join our team here at Community Action: MK as the Integrated Discharge Hub: Voluntary and Community Sector Coordinator.
The Integrated Discharge Hub (IDH) at Milton Keynes University Hospital (MKUH) is an initiative that has been set up by Milton Keynes University Hospital, Milton Keynes City Council, and Central and North West London (CNWL) NHS Foundation Trust. The IDH is made up of a diverse range of specialists working together to support patients to transition from hospital to home as smoothly as possible. The IDH helps people to receive the care and support they need to regain and maintain their independence; hopefully preventing future hospital admission.
This role will enable MKUH to effectively support patients referred into the IDH to access services and activities within the voluntary, community and social enterprise sector (VCSE). These VCSE led activities and services will help to meet the needs of individuals following discharge. The overall aim of this role is to increase access to support that helps patients leave hospital efficiently and stay well in their communities.
We are keen to hear from a range of people with relevant skills and experience, whether they were gained via education, volunteering or paid employment. To be successful, you will need to demonstrate that you are organised, can work both independently and as part of a team, and are passionate about health and wellbeing and the VCSE. This is a vibrant role that will involve a number of different responsibilities, including outreach to groups and communities, coordinating training and events, identifying relevant VCSE support options for patients, facilitating meetings, managing communications, information and data, partnership working and gathering intelligence and insights.
This role is being funded by health and social care partners, working with the VCSE – Milton Keynes City Council, Milton Keynes University Hospital, CNWL and the ICB.
